چکیده

Fransen et al. [1] published a Heart Beat in this journal showing a case with rate-dependent left bundle branch block (LBBB) in a patient with serious hyperkalaemia. Critical appraisal of the ECGs, however, suggests that the hyperkalaemia by itself may be the explanatory mechanism of the LBBB rather than the increased heart rate. As shown in Fig. 1, the QRS complex immediately following a prolonged RR interval (10th QRS complex) is still widened, ruling out rate dependency as the underlying cause of left

Rate-dependency of functional block in the human His bundle and bundle branch-Purkinje system.

SUMMARY In eleven patients in whom atrial premature stimulation elicited functional block in the conduction system distal to the atrioventricular (A-V) node, the influence of atrial driving rate was studied. Functional conduction disturbances in the left bundle branch-Purkinje system were produced by driving the atria at a rate just above the spontaneous sinus rate and by introducing premature ...

Alternating bundle branch block during atrial bigeminy.

Alternate patterns of ventricular excitation may occur with any type of supraventricular bigeminy. It is a physiologic phenomenon that depends on the rate dependency of bundle-branch refractoriness and the linking phenomenon related to concealed retrograde penetration of aberrantly conducted beats into their own blocked bundle branch [1–3]. An alternate pattern of ventricular activation can onl...

Electrophysiological studies in patients with rate-related intermittent left bundle-branch block.

Two patients who developed left bundle-branch block with increase in heart rate are presented. Atrial pacing during His bundle recording studies showed differing responses in the ventricular specialized conduction system of the two patients. In one, the onset of left bundle-branch block was associated with no change in the previously normal HVinterval (so msec), while in the second the HV inter...

Haemodynamic Benefit of Cardiac Resynchronisation Therapy Requires Left Bundle Branch Block: A Case Report

A 55-year-old woman with dilated cardiomyopathy and rate-dependent left bundle branch block had a cardiac resynchronisation therapy (CRT) device implanted. During implantation, the maximum rate of left ventricular pressure rise (dP/dt(max)) was measured invasively. This case presents a description of the acute negative effect of a left bundle branch block on dP/dt(max), and the different effect...

Prognostic impact of electrocardiographic signs in patients with Type 2 diabetes and cardiovascular disease: results from the PROactive study.

AIMS Although a resting electrocardiograph is broadly applied in clinical practice for evaluating patients with Type 2 diabetes and cardiovascular disease, the independent prognostic relevance of electrocardiographic signs has not thoroughly been examined. METHODS Baseline 12-lead electrocardiographs available in 5231 of the 5238 participants of the PROactive trial were analysed for heart rat...
